*COVID-19 Reopening Release Waiver *
COVID-19: the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing. Technical Soccer Academy LLC has put in place preventative measures to reduce the spread of COVID-19. However, Technical Soccer Academy LLC cannot guarantee that I, my child(ren), or anyone else will not become infected with COVID-19. Participating in a Technical Soccer Academy LLC athletic program or activity could increase the risk of contracting COVID-19. In consideration of being allowed to participate in any way in any Technical Soccer Academy LLC programs or activities, I understand, acknowledge, and agree to the following: 1. I knowingly and willingly consent to be coached by Technical Soccer Academy LLC coaches and staff and any designated associates and employees during the reopening phase of COVID-19. 2. I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms yet are still highly contagious. It is impossible to determine who has COVID-19 and who does not, given the current limitations and availability in COVID-19 viral testing. 3. Risk of transmission: I understand that due to the frequency of contact during the training and participation of soccer between coaches and players, characteristics of the virus, and characteristics of training with Technical Soccer Academy LLC, that I have an elevated risk of contracting the virus by simply participating in this training, even though standard precautions are being observed. 4. I am unaware of being a possible carrier or infected: I confirm that I have not tested positive for COVID-19 in the last 30 days and that I am not presenting with any of the following symptoms of COVID-19 I. Fever of 100.5 degrees Fahrenheit or 37 degrees Celsius or higher II. Shortness of breath III. Dry cough IV. Runny nose V. Sore throat VI. A diminished sense of taste or smell 5. Contact with infected: I confirm that I have not knowingly been in close contact (defined as 6 feet or less for a duration of 15 minutes or more) with someone who has tested positive for COVID-19 in the last 14 days, or with anyone that has had the above-stated symptoms in paragraph 4 (#4) in the last 14 days 6. Public travel: I confirm that I have not traveled outside of the United States in the past 14 days. I confirm that I have not traveled domestically by commercial airline, bus, or train within the last 14 days. 7. I commit to completing a daily, non-invasive temperature screening at entry and confirm that I have had no known exposure to the virus in the last 14 days. 8. I commit to observing the requirements around protective equipment, sanitization, and distancing communicated by the Technical Soccer Academy LLC staff and per the signage at each activity. Informed consent: I have been given the opportunity to ask any questions regarding the risks of contracting COVID-19 from Technical Soccer Academy LLC and participation in their programs and activities. I reaffirm that I am not a carrier of COVID-19 nor infected with COVID-19 to the best of my knowledge, including those in the household. I voluntarily assume any and all risks, including a substantial and significant risk of serious harm, which may be associated with any phase of my training as a result of the COVID-19 pandemic. I acknowledge that the nature and purpose of the training procedures recommend under the current circumstances and restrictions that have been explained to me and that I have been given the opportunity to ask questions. ; By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by participating in a Technical Soccer Academy LLC athletic program or activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself or family members. I understand that the risk of becoming exposed to or infected by COVID-19 at a Technical Soccer Academy LLC program or activity may result from the actions, omissions, or negligence of myself or others, including but not limited to Technical Soccer Academy employees, volunteers, and program participants. I understand and voluntarily accept and assume all the foregoing risks related to COVID-19 and accept sole responsibility for injury or illness that may occur. Further, I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Technical Soccer Academy LLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Technical Soccer Academy LLC program or activity.
